MODIFICATION
- Signed application from lender/servicer
- Signed additional forms/disclosures if applicable
- Hardship letter signed and dated
- Copy of recent mortgage statements
- Past 2 years tax returns/Schedules/W-2's
- Last 2 months of bank statements/other asset accounts
- Last 6 and/or 12 months Profit & Loss statement if self-employed
- Leases on all rental properties
- If listed for sale; provide copy of MSL printout
- Copy of Drivers License (all parties on app)
- Copy of most recent property tax bill
- Insurance Declaration page
- Any delinquency/legal notices
